Ashley Alvarez is a nurse practitioner at AltaMed Health Services, the nation's largest Federally Qualified Health Center. She serves as a Transition of Care Nurse Practitioner in East LA — working as a walk-in clinician to expand access to urgent and primary care for underserved patients. She built an in-clinic workflow linking Transition of Care, Enhanced Care Management, Viva Gold, and primary care teams that helped reduce ER and hospital visit rates. She precepted UCLA and Western University medical students plus AltaMed NP Fellows, and was named a 2023 Core Value Awardee for the ELAC region. Her path began at UC Irvine Medical Center as a nursing assistant and charge nurse before completing UC Irvine's TNT nurse practitioner fellowship in transition of care.
